First Visit to Heritage Dental
During your initial appointment your doctor will perform a thorough, comprehensive examination and take any appropriate x-rays and diagnostic study models. After studying each patient’s individual needs, your second appointment will be a consultation appointment to receive feedback on your priorities and a treatment plan will be developed. Treatment is planned with the patient’s desires, needs and expectations a primary consideration.
Please assist us by providing the following information at the time of your first visit:
- A list of all medications you are presently taking.
- If you have medical or dental insurance, please bring your card(s) and all subscriber information. This will save time and allow us to help you process any claims.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the initial visit. Please alert the office if you have a medical condition that may require pre-medication prior to any dental treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
X-Rays
If your previous dentist or physician has taken x-rays, please request that they forward them to our office. X-Rays may be faxed to 540-373-6830 or emailed to [email protected] If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
